By accepting this section and as father / mother / legal guardian / responsible for the registered participant, in the event that none of them is deceased:

I DECLARE that the registered participant does not suffer from any type of health problem that contraindicates the sporting practice of this activity, being aware that the safe exercise of the sporting practice requires frequent training and the passing of the relevant medical check-ups relating to the medical-sporting aptitude of the participant. I also declare that the information regarding the participant's state of health provided to the Club on the registration form is true. In the event that the participant's medical situation changes before the start of the activity, I declare that I am responsible for notifying VALENCIA CLUB DE FÚTBOL S.A.D. by e-mail campusfutbol@valenciacf.es for the appropriate purposes.

I EXPRESSLY EXHIBIT VALENCIA CLUB DE FÚTBOL, S.A.D. and all the organisers, sponsors and collaborating companies, as well as all the staff involved in carrying out this campus from any liability arising from events arising from the participation of the minor in this activity, expressly waiving any claim against the persons indicated for any damage suffered by the participant arising from the inaccuracy of the statement made in the previous section.
